Community Inclusion means that all people, regardless of their abilities, disabilities, or health care needs, have the right to: Be respected and appreciated as valuable members of their communities.

Cornerstone Mentors of Oregon welcomes participants to develop life skills and take part in the community.

Supported Living

We offer assistance with Activities of Daily Living (ADL):

  • Assistance with light housekeeping including things like dusting, decluttering shelves, and vaccuming;
  • Personal care including things like advocating for emotional care, personal and family time, hygiene, and meal preparation;
  • Financial budgeting includes things like working with individuals to find the right resources to manage their financial resources;
  • Appointment assistance including providing help with using technology such as telephone, email, or other electronic means to assist individuals with their daily needs.
  • Community Inclusion

    Community inclusion is essential because: It enriches the lives of our family members; it is stimulating and addresses their need for human interaction. It is a means to skill development for potential employment and daily living. We offer assistance with:

  • Transportation assistance including how to the buses and MAX trains, information on transportation choices, personal trip planning, and driving services. Providers, with registered vehicles, vehicle insurance, and maintained (in vehicle) first aid kits, are available to provide transportation as needed;
  • Cognition and assistance with community activities including things like companionship, meaningful activities, and conversation;
  • Assistance with awareness of environmental safety hazards including sharp objects, traffic and crosswalks, appropriate lighting, and any other environmental hazards that may be a risk to the individual’s health and personal safety.

    Meet a Mentor, one-on-one peer interviews are available. Interviews take place at 11 a.m. and 2 p.m. Monday thrugh Friday. All interview must be scheduled in advance with CMO's Administration office.
